Yes, this is intentional as wearing underwear underneath swimwear is unfashionable!

Unfortunately, this is at the discretion of the author. Sometimes mentions can be easily added, but not always--just depends on the scene itself. I'm sure it wasn't omitted on purpose though.

Like above, it depends on the author's intention--or if it was overlooked in any way. In this case, it seems to be a pure physique check for physique training (slow stat gain). The difficulty check is supposed to be pretty hard, so, instead of bypassing it completely, I've only added a modifier to make it a little less difficult if the character can fly (flight-capable wings, levitation, jetpack, etc.), but not by too much, hopefully (I didn't make any mention of flying variations in the text though). This should keep the original purpose intact. Given, if this is unbalanced or unfair, the change may be reverted.
