After character creation, it appears only pregnancy/cum can change belly size, and that base belly size is static. Increasing or decreasing tone and thickness, which should cause belly size to be recalculated, does not cause this. Hornet Sting, which the Wiki suggests sets Belly Size to 7, does not do so.
