You didn't activate the Self Destruct during Kara Quest 2? You should probably upload your save. It's basically impossible to diagnose exactly what went wrong from your description.

Can you give more detailed instructions on how to reproduce? If you're relying on Draining, that only works on enemies that still have Shields.

None of the X Chance flags affect enemies until they're doing HP or Lust damage. Special Attacks (i.e. Paralyzing Shock, Overcharge) can inflict status effects through Shields/Armor. I'd still recommend Stun or maybe Vampiric. A surprising number of enemies have no/negligible Shields. For those...

I went and tested this and it's weirder than that. Sometimes she does, and sometimes she doesn't, even with a pure male Steele.

Her first appearance is a likely, but not guaranteed event when you exit the Xenogen office. (Or load a save on that square.) If you still can't encounter her, it might help to upload your save for testing.

I'm pretty sure it's just a side-effect of the way it's implemented. Debris Field behaves the same way.

You can still do the uplift quest. (IIRC, it involves getting Dr. Lash to create a distraction so you can sneak into her lab.) You could us the Cheat Menu to teleport into the room, maybe. There's always Save-Editing.

Dr. Badger has a Silicone Tank that can be used to remove Silicone, but I'm pretty sure you have to uplift the Pexiga first.
