Is it me or is this fight absolutely rigged every hit penetrates my shields and does 50 direct to health damage and I'm down in like 2 hits

Like all such posts, we need to know what your build out is. My Steele has no problems, but as a level 13 tech specialist, she has little problems keeping her enemies stunned with charged weapon + the magma crusher. Also, stock up on EM Poppers on Dhaal - they stun all the enemies in on the train. Timed with the crossing signs, they can deliver a nasty punch - their own damage plus the crossing sign damage.

What's your Class?
What are your Perks and Stats?
What is your Equipment?

She has 60 Physique, 40 Reflexes and 45 Willpower.
She should be fairly vulnerable to disables (Headbutt/Paralyzing Shock/Flash Grenade), so long as you have the stat maxed.
Mag Binders with 60+ Reflexes is guaranteed to Stun.
Gravity Flux should reliably Trip her, because the Trip targets Willpower.
Burn Chance should be fairly effective, though she has 25% resistance.

If you head South, you can do an optional fight that will let Johana help.

Have you tried Paralyzing Shock? It should be almost a 50/50 chance. If you take a Focus Pill, the odds should be slightly in your favor.

Are you fighting with Johana? That would give her another target.
What Perks did you take? The Turret would be a great distraction. She might try to Grapple it, instead of you.

Activating the Cheat Menu lets you refund and switch Perks.

i managed to get through it with P shock and my usual tease attacks, the other bosses were all basically immune to tease so didnt try it until now

i should not have to use easy mode a attack that cant be dodged that does direct health damage through shields repeatedly is bad design
Easy modes are a great way for players, that are bad at video games, to being able to enjoy them too. Most just don't realize that they're bad at certain games and feel attacked and offended when using these modes is suggested to them, as if playing games on "easy" would somehow make them lesser as a person.

So I don't blame your unwillingness for turning the mode on and that you would rather resort to saying that this easy enemy encounter is badly designed.

Anyways, I wish you the best of luck with beating her!

It can lock into a permanent loop of direct health damage, get grappled, break free, direct health damage, grappled again all without ever getting a turn until you loose

Yeah, you convinced me now. It somehow happened to you repeatedly - I would assume judging by this threads title and contents - while you are not a bad player. I'm sure your characters stats, gear and skills are completely fine.

So the solution is to tell people to turn on Easy mode, instead of helping them get better? Maybe there's only so much you can do in an action game, where reflexes matter, but this is a turn-based game. PC stats and abilities matter more than the player's physical abilities. This fight can be trivialized if you know what you're doing.

It can lock into a permanent loop of direct health damage, get grappled, break free, direct health damage, grappled again all without ever getting a turn until you loose
If she can Grapple on the same turn you broke free, you might report that as a bug. It should probably have a short cooldown. Actually, looking at it, it's supposed to have a cooldown.

The fact that it auto-hits is also potentially a problem (though you can give her another target). Your 20 Reflexes isn't doing you any favors. (You have 8 extra evasion vs her 24 extra accuracy.)

OP didn't make such a request. I told them how to beat the fight. Nothing more, nothing less.

If you on the other hand wanna help them get better, then more power to you! :)

That's why Sense is in the game.
Oh no there literally just resistant to tease I have widows kiss, should normally do 6 tease damage and it increases their count by 1 also heavy slut ray is an increase of 1 also

No, I mean Sense will tell you what the target resists/is weak to.

For example, the Warbitch has ~70% Resistance to teases. The Warmistress has 15% weakness.

Also, you get 1/4 Physique to Widow's Kiss Tease (Phys/2, divided between damage types).
But NPCs get some base resistance from level on top of the bonus from Will.

22.3 - (9 (Will) + Level bonus)

A lot of the time it doesn't matter because you can brute force it anyway.

I do recommend filing a bug report on the chain-grapple. If I'm reading the code right, it's supposed to have a 7-round cooldown.
Include a copy of your save, if you have one before the fight. And maybe a screenshot of consecutive turns.

Generally, flying (either with a jet pack or innately, like my half-Gryvain Steele) will cut a lot of the grapple attacks. Pippa's slippery oil massage perk is also good for shutting down grapples, as is Paige's Limber perk.
In the specific case of the Warmistress, Ero Gas grenades are also very useful for quickly maxing out lust.
