Will we ever be able to have sex with Nelia?

Is the giant half Lupine mentally ill? She doesn't talk even though both Giants and Lupines are intelligent. And she just walks around in the woods wearing a leash and lives in a cave.

Why would she have to be mentally ill?

Maybe the girl has lived by herself for so long she just doesn't have the ability to talk? Now, she can still communicate, understanding what the Champion says to her.

As for the leash - she has a kink, simple as. And the cave? I can't imagine a house suitable for a half-giant would be the easiest to build properly.

I assumed she'd die alone if she was just left there as a child. The house thing makes sense though. Unless she moved in with those horse people or the minotaurs, finding a house that'd fit her would be pretty hard.

And isn't Mutism considered a mental illness?

Read about feral children, you'll get a better idea of how someone can be isolated and raise themselves from a young age.

As for the muteness, no, muteness is not a mental illness. While selective muteness is an anxiety disorder, its also defined as "someone normally capable of speech cannot speak in specific circumstances". But that's not the only kind - there's muteness caused through physical injury, through disease and simply never actually learning because they were never exposed to it growing up.

So don't go slapping "mentally ill" around the place without doing a little research first.

According to Mayo Clinic, anxiety disorders are mental illnesses.

The feral children thing is crazy interesting though. The fact that human children can be raised by dogs, wolves, and even ostriches is insane. It's pretty sad that most of those kids end up mentally and physically disabled.

Unless her author can expand on her origins, she's pretty much a giant enigma... There aren't a lot of giants out in the world and we're not sure where she originally came from... she seems to live pretty comfortably since she can survive despite being essentially a giant pet.
