Is the only way to obtain this perk by Sera's questline? And if so, why is it locked to people with Ditz Speech?

You can also potentially get it if you've submitted to Lumi enough that you sometimes auto-submit to her. That said, be aware that continuously auto-submitting to her at some point starts giving you the Dumbfuck perks, and will eventually cause a bad end after you obtain the Ditz Speech perk and keep doing it.

If you'd rather not permanently change your character, there is an item you can wear to give you ditz speech temporarily called Symbiotic Bimbo-Hat. You can get it from Kiro Quest or just use the cheat menu to add it to your inventory.

You can also use Doll Maker in Dr. Badger's office after the Pexiga's Uplift quest to temporarily give you Ditz/Brute speech for about 2 days, I think.
