You didn't activate the Self Destruct during Kara Quest 2?

You should probably upload your save. It's basically impossible to diagnose exactly what went wrong from your description.

"KQ2_NUKE_STARTED":957531
"KQ2_NUKE_EXPLODED":1
"KQ2_MYRELLION_STATE":2

The game certainly thinks you set off the Self Destruct.
I've taken the liberty of removing the flags.

From what I recall, it is on a base that is very close to the planet. After the bomb on the base explodes, and because tensions are already so high, the governmental forces that are orbiting the planet (to manage the truce between the myr factions) perceive it as a threat, so they end up nuking the planet to prevent more war from breaking out. Do correct me if I'm wrong though.

The journey toward the icy blue moon isn’t particularly long, by galactic standards, but the coordinates Kara gave you lead into the rocky belt encircling it.
The trip goes by quickly and peacefully, until you hit the arid atmosphere of Myrellion.
A short while later, you find yourself staring into space. Your sensors show the Confederate fleet in the distance, and you’ve had your sensors tuned to detect weaponsfire in the nearby system. But so far... nothing.
Just as you’re starting to wonder, your sensors detect movement in the fleet. You watch with wide eyes as the Confederate fleet moves away from the planet, making way towards the Warp Gate. They vanish off your sensors an hour later, warping out of the system.
Well, that doesn’t bode well for the cold war down there. Indeed, as you sweep towards the planet’s surface, your sensors start to blip up with various radiological alerts. The closer you get, the better you can see mushroom clouds billowing up through the planet’s atmosphere.
You meet Kara on an asteroid orbiting Myrellion's second moon, but the Black Void base is on Myrellion.
If you set off the Self Destruct, the Confederate fleet leaves, but the Myr nuke each other.
