This might not actually be a bug per say, but an inconsistency. In the game options you can edit your character with the character editor to give them a hyper-pumped anus. I have a separate test file from my main one to play around with various character edits to see the results, and I do like the little "doughnut" bit in the character description screen when they have the hyper pumped anus trait, but my main save file is clean and edit-free with no cheats enabled. I'd like to be able to naturally give my character a hyper-pumped anus at some point in her adventuring career, but the only transformative options that really increase size is Anusoft, which can give you a slughtly-pumped or a fully-pumped anus, but not a hyper-pumped one, and I'm unaware of any scenes my steel can encounter that would naturally afflict her with the status. Perhaps there is one out there that i don't know about, but if there isn't then that would mean there is no way to naturally generate a hyper-pumped anus, and almost certainly none with no additional side effects.
I believe anusoft was implemented before the hyper pumped status came out, so the transformative never had any conext beyond fully pumped. I suppose this is a request for the content creators more than anything.
