The Champion needs to:
a) have a Real Cock (EDIT: and not be impotent) and
b) have completed enough of Nakano's quests to have received a visit from Lord Keros.
Champ does not need to be a kitsune or to have freed Kiyoko in order to unlock it - they need to have the option to be a kitsune, whether taken or not.
Once those requirements are met, Champ should visit Miko from time to time and it'll proc relatively quickly.
For what it's worth, the scene is dramatically different depending on whether Kiyoko is there with the Champion.
