To those who decides to marry someone in the frost marches or the old country, how does the family name business works?

does marrying Kiyoko being a matrilineal family means you become (PC) Ōtomo? Or Nakano will become Nakano Ōtomo in the future?

Do we Know Brint’s family name?

Unless it's brought up somewhere that I've overlooked/forgotten with Kiyoko specifically (where there would be precedent in the Japanese culture the kitsune are based on for the Champ/Kinu's husband taking the Otomo name) I don't think it's ever addressed. Given the large number of people the Champ can or will be able to marry (all at once potentially) and how the Champ may or may not have a family name of their own at the start and may or may not be in a position to take a new one if they become a local noble, it's something I doubt they're ever going to address and they'll just leave that up to individual headcanons.

I don't think anything's been said about Bri's family name, or if they have one.

Yup my champ will only have one family. And I’ll have his last name Ōtomo unless the devs change it somehow.

There is plenty of room if they want to expand that area, for multiple characters too.

Most characters (including most Champions, based on their background) probably don't have surnames, and different cultures have different customs regarding them where relevant, and those customs may change even within a culture based on social strata. Family names of the royal families are probably going have more weight behind keeping them in use as compared to that of a commoner.

In game, we probably won't touch too deeply on it, for the reasons Wolfram provided.

History tells us that in ancient times, noble and reigning families received the surname. Also, the surname could be granted along with the title to particularly distinguished ordinary people. Over time, surnames began to appear among non-noble people to denote succession or to denote the owner (if we are talking about slavery or similar systems of human ownership). For example: Ivan Deryuzhkov in Russia 18xx meant "peasant Ivan in the ownership of the landowner Deryuzhkov". Since CoC offers us a setting (in general) of the Middle Ages, then the rules for obtaining surnames are generally the same.

tl;dr - surnames are for nobles and kings/emperors, not for the ordinary folks. For the tribal system (like Centaur village, Kervus tribe, Marefolk village etc), the correct form will be "X, son/daughter of Y", but not the surname.
