After talking to Lusamine about her fertility issues i haven't gotten the quest, is this locked behind a certain amount of progression or is my save busted?

Yes there are. You need to ask her about her ("About Her" in her talk options in Frosthound) and you need to have intercourse with her in the field.
Then in her talk menu you should see new option: "Sterility".
Beside that you also need to defeat Jen more than 5 times and see Leoric. This allows to continue the sterility talk.
For future reference this should go into general questions not bug reports.
