Step 1: Progress far enough in the den to hear Keros offer to turn you into a kitsune (scene occurs near the waystone). You do not need to accept, just to hear it.
Step 2: Bed Kohaku at least twice (If done before Step 1, bed her again).
Step 3: In the aftermath of a sex-scene, you'll see an additional "dreamlike" scene, in which Kohaku is initiating another sex scene. You will be warned that "if you don't wake up soon, this will have more consequences than a mere dream would".
Step 4: Select "No" (As in, "don't wake up")
Step 6: Wait. Kohaku should confront you about the pregnancy you caused in about a week.
