I've noticed on some of my male characters it'll say I have a "flat, girly chest". What causes this?

By my reading of the code, this happens when your character is androgynous and has a low muscle tone.

That's interesting. I have a femboy character with max femininity and low muscle tone and I don't see that descriptor.
I have preferred gender set to Male.

I think setting preferred gender over-rides the auto-detection. It may also be that you are seeing another one of the options - the description is chosen at random between three options.

Most of the descriptors for the appearance screen are taken straight from TiTS and as mentioned a number of things affect it - namely femininity, muscle tone, and gender.

Early on there was an adjustment to the parser to avoid weirdness like a max masculinity female character being described as cute/pretty but the appearance screen descriptions aren't affected by this. You could ask Upcast Drake about it.
