Generic human pregnancies

So far, either one or none; all human or originally-human NPCs that can have kids with Steele only have specific pregnancies. The only generic pregnancy that results in human children is the sydian one, due to sydians being an offshoot of humans, which would've been explored in a quest that never panned out, but the text for that is still present in the code (unless it was removed in the JS conversion) so I still consider it to be canon.

My character got pregnant with human kids when she didn’t fuck a human, also somehow one human child is a herm

NPCs make kids based on their original race, not their current one. The only exception I know of is Haley, who'll make kids based on Steele's original race.

Then how can a futa human be born?

I dunno. I guess that's an oversight with that particular NPC's pregnancy.
