Doubt about the combat system

As far as I know, when you want to hurt someone in this game, it works like this:

1. To hit: Accuracy roll vs Evasion/Physical Resistance/Magic Resistance/Mental Resistance
2. Damage reduction #1: Penetration (Armor pen, Spell pen, Temptation) vs Armor/Ward/Focus.
3. Damage reduction/enhancement #2: Type resistance (ex: fire resistance).
4. Damages dealt.

Am I right? Or am I confused?

Some more questions if you (or others) don't mind:
1. What is the Accuracy roll needed to Crit (outside of chances provided by your Crit chance stat) 100+ or 120+, I've seen different sources (the in game codex says 120, but it also mention Resolve...)
2. What determines the Crit chances of Healing? Your Crit chance only? Or is there some kind of Accuracy roll too (which would be strange, since heals always hit their target)?
3. Does a shield protects you against 'on crit' effects or only against the enhanced damages?
4. Do we have the details on how Threat generation works exactly? Attacks and heal seem to build Threat, but are there details on that somewhere?

1. 120+. It used to be 100.
2. Just your CritChance.
3. I believe it prevents crits from triggering at all rn.
4. Basically you just gain Threat equal to the damage or healing you do, sometimes modified by a percentage granted by powers or perks or items. There's no cap to Threat, and it's a completely independent number for every character.
