Difference between acquired and character creation perks

In the wiki, some perks are listed as being obtainable both during character creation and from transformation items. Some of those also have a text that says that the stat changes are applied only in character creation, but I am bit confused whether it applies to all stat changes, or just the ones listed before the text, or to the changes listed after.

For example:

Messy Orgasm

*If has a Vagina:
**Raise Vagina Wetness by 2
**Raise Raw Cum Multiplier by 1
(the stat change is only applied during the character creation perk choice)
*Increases Cum Multiplier by 2x base amount
*If has a Vagina:
**Classified as squirter
*(+1) Fame with the Temple of Mallach

For context:
I'm trying to create a character with maximized cum output, with secondary priority being virility.

If the character creation perk is the only one that increases the base cum output, then I think I should choose that one. If that base cum output increase is also achievable with transformation items, then I think I should opt for Breeder to boost the virility instead, since that perk cannot be gained with transformation items at all.

But I can't quite figure out which way this works, so any help would be appreciated!

There are some perks you can get ingame aswell from the character creation options.
See in the Perks page under the Sexual Perks, for the messy orgasm you could get it ingame through using the Incubus Cum Transformative Item.
Hi, thanks for the reply, and yes, that's right, transformation items do give perks.

But that page explicitly says that the stat changes are only applied at character creation, for this perk specifically (and few others, too). See my original post for the exact wording.

That particular text is in the middle of different stat changes, so I do not know does that statement apply to the stat changes before that text, or the ones after it, or both.

Dunno about that, but for me it would no make much sense if the ones at the character creation menu were to affect your PC differently than the ingame same ones.
