Cultist seems to die without actually getting hit

There have been other instances where they took like 48 damage on 50 ish health and still died, but I wasn't sure if I'd just misread their health. But this guy just flat out died from nothing. On steam and dark difficulty if that's relevant. Save is edited, but this has never happened before, and I've not changed anything I hadn't done before, so I doubt that's the cause

if that's the fire spirit, its attack display is bugged. It's a dual wielding attacker but it only ever shows one attack in the log. The other attack it made that didn't display must have finished it off.

If you look at their attack card, it shows they're attacking with two weapons.
