I'm trying to get Kiro's quest, and the wiki says I need to be level 9 plus (I'm level 10), at 100% trust (I don't see how I can't be), and not recieved the distress call yet (I can still meet her at the bar so she doesn't seem particularly distressed yet) and I've flown about 15-20 times, but I haven't gotten it yet, so I was wondering if there was some weird thing stopping it, or if it's just up to RNGesus
Any hidden requirements for Kiro's quest?

After checking, her trust is in fact at 100%, and I don't know if I'm eligible for other distress calls (had a look earlier but couldn't see a wiki page for them) but I'm certainly not getting any
It may be possible you have a state that prevents any distress calls in general. There are a few things that cause that (I think), but I don't remember what they all are.
Maybe turn on Halloween via the Cheat Menu and see if you can't get the Event Whorizon or Kashima Incident (if you haven't done it already)?
Did you start (but not finish) Kiro's date on Canadia? That might block it. I got the call after getting and reading the e-mail, but haven't visited Canadia.
Are you getting any Pirate encounters?
If all else fails, you can upload your save so someone else can take a look at it.

I believe I've completed the date, I tried waiting in the bar for several days to see but Kiro didn't want to show up
I haven't gotten any pirate encounters for a while, but I did get that one encounter where you race, so there's that
I don't really wanna mess around in the cheat menu (mostly cuz I have no idea what I'm doing), and if push comes to shove I probably will upload it here
The Cheat Menu is designed to be easy to use. If you're afraid of messing something up, just use Save to File to create a backup.

I assume you haven't already started it. because you'd be getting status messages for a while, which you're unlikely to miss.

I ended up meeting Kally and becoming "official" with Kiro, even had her and Paige meet, so I figured I had everything completed
I'll have a look at the cheat menu soon and test
It's been a while and still haven't had it trigger, so I decided to try Halloween and I can in fact get the Event Whorizon to trigger (note I used Silly Mode)
Thinking back I haven't had any random pirate encounters since I posted this

Do you perhaps have Paige set to avoid pirate encounters (Paige -> Navigation if my memory serves)?
I did yeah, but I didn't think it made the chance zero
I also believe I didn't actually have her when I made the post (though I very well could be wrong)

I loaded an old save right before I tried for the po mission.
Got trust to 100 jumped around no luck.
Finished kally hookup with the glory hole scene followed by a talk with kally jumped around no luck.
Then I kicked Paige out of the ship jumped a few times and got the message.
