If your PC starts with the [Reserved] perk, they'll have 0 Libido and seemingly won't get the Blue Balls status effect.
Although, I think that the threshold for the Blue Balls status effect might be >=10 Libido (10 Libido being the starting Libido for non-Reserve / non-Libidinous perked PCs), hence why the Reserved perk starting PCs at 0 Libido would seemingly negate the Blue Balls status effect.
Notably, Ice Wine could be used to the same effect as the above (Reserved perk 0 Libido shenanigans) - and therefore would also allow for the PC to avoid the Blue Balls status effect.

About that...
I've been sleeping non-stop to progreas Kiyoko's content because nine tails nonsense and eventually my blue balls debuff just stopped showing up after a week or so of that - pretty sure I've passed like 21 days of just doing this so far - I did do a Cait & Fuck with Etheryn on like day 2 of that first week though, when it first did show up; but it never appeared again since.

Literally just loaded your save, skipped out on sexing Kiyoko and the status effect popped up instantly. Sex with Kiyoko (although in the dream world) still counts as sex, which means you won't get the Blue Balls status effect if you do so.

That's weird, because it's procced Blue Balled on meeven after playing around with her; I've even moved around between tiles after Kiyoko fun and it never procced for me...
