Victory -> Ride Cock -> Vagina -> Take Inside
Scene Ends and there is no filled status

Tried the same thing with the Salvager and same result - no vaginally filled status

Tested to see if all status were broken but it seems to be only the vaginally filled one.

Going into the stats menu also shows no cumflation status either

It looks like nothing is wrong with your file or the game. If you check your character's medical stats, you'll see that the vagina's womb is already pregnant. Because if that, it can not take in and hold any extra loads (because it's making retaining space for the baby). If you chose to take the load up the butt, the character should correctly undergo cumflation, so it seems everything is working as intended.
