I am consistent targeted by mobs and bosses even though I'll place one or two companions that generate aggro. This happens regularly to the point that having foes attack other characters that aren't mine is like hitting a crit. My strategy has been boiled down into rushdown then heal. Here are some examples of this but this has been a problem since starting the game.

It's not a problem with agro, as a very very long time player. Aggro is easy to manage with even small threat management, it's entirely a loadout/skill error on your part. The only one I'm convinced might have been reasonable was probably the first one with the Vulps, because they have Mark for Death which makes their target generate a large amount of threat, tank or not, so if they use it before the tank secures threat or you go nuts on them before threat is established, your gonna get cratered.

We can't give you any other advice since we don't know your build or anything outside of that you're a spellblade, which is pretty notorious for generating high threat early and often due to Charge Weapon and not waiting before doing high damage attacks.
