There is no RNG involved. Soon as it's been 14 days without her being milked and she's around she'll let you know. Were you passing time by sleeping or waiting? You have the Brienne + Azzy sleep enabled, which milks them both in the wake-up part.

For the Quin scene, it's a 7 in-game day cooldown. So it can repeat. For Brienne, I was able to get the milking scene to proc. It currently takes 14 days without her being milked for it to proc. It happened on day 979 with your save for me.
