I've had it proc without any followers, I think you need to have Fetishes turned on in your settings, I had it on one time by pure accident and haven't been able to reproduce it since turning it off.

@Stadler I was more a long the lines of referring to the Jörmungandr from the Monster Musume universe

While on the topic of Reptilian races I was kind of thinking of Jörmungandr, but I am not sure how that would work since it is essentially a Lamia (I am talking about the actually meaning for Lamia which in CoC are Nagas) but that are much more massive.
