This is probably because nobody really fucks the Hydra, but apparently the pregnancy is busted beyond the belly increase for flat characters. Unsure if this happens anywhere else. Attached is the save file.
Fixed Zaika Eggnancy Fail

The intent was for the character to grow their breasts, but the code implemented it to work only for non-flat characters. This change should now allow flat characters to grow breasts. After the first growth, each stage will regrow the breasts if they ever become flat throughout the duration of the pregnancy. Because the growth is permanent, after the pregnancy ends and you want a flat character again, you'll have to find any items, events or services that will reduce the character's breast size.

How do you trigger that? My Steele's done about everything with the Hydra she can, with no eggnancy. Is it only for certain races? Is Breeder's Bliss needed? Is it only vagina/womb #1? (at the time my Steele had #1 and #3 full, but had #2 open).
From what I can see, for the Zaika Hydra it happens at random on loss (choosing to "Fuck", and not "Fight", when encountered also counts as a loss). Having a large penis or a parasitic tail cunt will decrease the odds by a lot, but there's still a chance as it is not gated. Of course, having a vagina will be needed for the pregnancy, vaginal will take priority over anal, and the hydra will choose a random vagina if you have multiples.

As Jacques explained, that's what you have to do. Idk if Breeder's Bliss increases the odds though, I had to seriously grind to get a Zaika even with it on.
OK, if I may make a suggestion for consideration: It Would Be Nice If a victory would allow for a choice that would lead to an eggnancy, or at least a reasonable possibility of one, no matter what other equipment Steele has, so long as Steele has a fertile womb. That is, allow the player to choose an action, and select a womb.

I feel like if you've forgotten which wombs are pregnant, that is a skill issue seeing you can get all three pregnant rather quickly. But would be nice for the self--inflicted Heat debuff which doesn't go away until all wombs are pregnant.
