Why does the design document specify that the player will always have some clothes on?

Hm, are "effectively nude" outfits still possible? like, say, running around clad in just jewelry.
Every single thing I write in TiTS - EVERY single thing - requires a variant for the leadin because it's possible for the PC to be nude. So I can't just write a stripping scene where the PC slowly removes items of clothing. I have to also write a variant where they start off nude and frankly, starting off nude can sometimes be fucking weird, like you really shouldn't be able to be nude in certain places. Eventually you don't bother with fancy leadins because writing them is a pain when you are also guaranteed to have to write an already-nude variant that has to have alternate text to explain what the PC is doing instead of taking off clothes. It makes writing a hassle much in the same way being a naga or a taur does, which aren't available as options to the PC for the same reason.

That does sound like it's fully possible to have "nudist-adjacent" clothing though, right? Like crotchless panties and nothing else, or the above mentioned jewelry gear (might try my hand at contributing something by writing that actually...)
One thing to note also is that the world doesn't start like the sexy apocalypse Mareth was, so running around the buff is going to get someone going "Oi, whatta fook ya doin' mate dinnae ye mum teach ya about knickers?"

Let me reiterate this, it's cold, really, REALLY FUCKING COLD where you are. At least from my understanding this "World" is a winter type world where most of the time it's cold as fuck. I could be fucking wrong, I am occasionally. (So sue me I'm human just like everyone else here
) Plain and simple do you really wanna suffer frostbite by not wearing anything, I mean it's logical to wear clothes. Also don't give me that BS about "Well I gots magic and I can cast it around me so I don't freeze my naughty bits off." Guess what magic costs mana, mana by the way isn't infinite.
I mean, the entire world isn't a wintery wonderland but the region the game takes place in is basically the alps.
also the best decision in the design doc is to forgo the whole fuck-all-the-shopkeepers thing. That always gave TiTS this fetish carnival feel that felt less like discovering interesting things and more like walking around to look at all the attractions. The problem with that is that it doesn't really feel much like a game, that the overlay is just a framing device for finding smut.

If you just want endless novelty a fetish carnival format is fine but that doesn't make it a game and it doesn't lend to cohesiveness; it's literally set up like a theme park where they'll have different sections (or sextions) with a vague theme and a bunch of mildly entertaining experiences to have that may or may not add up to anything.
The game overlay isn't necessary at all with that sort of format and progression through the game aspects isn't at all related to the smut. It could be a visual novel or anything else with little prompts to move on to the next isolated little scene. I could see right away from the design document that this, rather than trying to be a fetish carnival smut dispenser like TiTS, is striving to be a game first but also to directly combat that theme park feeling that unrelated fuck-things-just-because attitude creates.

It's a smutty adventure game. The description there is pretty apt, I feel, and wide encompassing for the mood.
I might even be so bold to add epic adventure, really. It'll be different from CoC's more "run around wildly" style and people who wanted that sort of... eh, "turn your brain off" empty, sexual festival sort of thing will probably be disappointed in this, as it's more structured on a similar scale to TiTs. Still, there's a best of both here, too.
