44.2 inches, though it takes a lot of metagaming; Well Hung + Minoblood leaves you with 31.2 inches, then heading to fuck the Centaur Pack Leader four times adds 3 + 3 + 6 inches for 43.2 inches (make sure to TF your dick to non-horse before doing so or you won't get the third TF), then trigger Kasyrra's valentine's encounter in the Old Forest which adds another inch.
EDIT: Actually Incubus Cum will give you a bigger dick than Minoblood with Well Hung, capping to 34,5, so that makes the final max size 47,5 inches.

I don't know what you can realistically fuck with that, seeing how at around 12 inch the game goes "can't fit more than that" or "can't break the limits of a humanoid body" for a lot of characters

There's actually a lot of scenes that even though a previous part of the scene description mention you not being able to fit any more of that monster cock in there completely forgets about it and lets you knot them at the end anyway.
But yeah, I got tired of most of that a while ago. Being a huge minotaur that could realistically only fit the tip of their dick in the majority of their non-giant conquests gets old quickly. My preferred size is actually a good old footlong these days, which is big, but still within realistic expectations, and normal enough for Brint and Kasyrra's grandiose dick descriptions to still have some meaning instead of my "conjoined twin" character wondering what's the big deal with these sissy peckers (though Evelyn's and Eryka's "massive" peckers even then have to be sort of ignored... Oh, just in case someone wants to take the whole hyper dick to another level entirely... While experimenting with my tentacle demon dick mage, and accepting all size increases offered by Kas and others (I mean, according to the description it shouldn't really matter, I should be able to reshape the tentacles to be whatever size), I remembered I hadn't done the Centaur Pack yet and went to fuck them for that extra +12 inches, because why not.
Turns out that Centaur TF does not remove your existing cock tags, it just adds the horsecock ones. You want to end up with a bushel of 40+ inches of knotted prehensile horsecocks, that's how you get it. Never mind the logistics of how you'd even carry that around without being just a ball of dicks, I can't imagine the faces of anyone who'd see those things erect coming at them...

The Tanuki Witch (aka Meira, who you can meet later in Evergreen's den when you have gotten Evergreen's bag back, and then get more precise alterations to things for payment) will only ever increase your dick to 30 in max, even if you have Well Hung, after that she'll only inflate the size of your balls. The Centaur Chief will add +12 inches total to whatever else you may have, with no upper limit, but it's a one-time deal once those three TFs have happened. So something else happened between those events, or you'd be 42 in.
I love cumflating various characters, but as soon as testicle description go up to "watermelon" or "barrel" as descriptors used, I'm out of here, lol, that sounds gross as absolute fuck, even compared to a bushel of horsecocks.
I wish there was a way to get quad balls like you could with Succubus Delight in COC1, then my Orc could have a Quad on them like a Krogan, and a slightly less gross way to cumflate someone. Though hiding it away like Meira can would be even better.

I usually keep my Champion's dick a cool 6-8 inches, I get a lot of compliments from a few fem NPCs but I think Hethia said something along the lines of: "not pathetically small, not egregiously huge, just boringly average." Which...describes my champion's fella pretty well--but still, ouch.
You can get 'nuki nuts from Meira @Sacremas (I think it removes 'refined' tag that I keep on the balls) but yeah, you can get that and it boosts cum production twofold without having to sacrifice aesthetics if you didn't know. You don't even have to be a tanuki. You can also get your ball efficiency boosted as well. Edit: It does make your balls huge if you aren't emptying your nuts, but it's temporary.

Yeah I used Nuki Nuts on my charmer, I think I had it on that image I posted with Lumia where it "just kept cumming" over and over, but if your production goes up high enough, you'll bounce back up to "barrel" and way bigger fast enough you need to play with that high pressure cumcannon between your legs one every couple of hours pretty quickly. Huge balls past a certain point just puts me in mind of really terrible infections and similar pretty much automatically, it's as nope for me as cervix penetration and womb-fucking is for women.
For the most part if I want someone with superhuman volume I just save edit the numbers after maxing what it could do in-game, bring it up to 10k or so for 4-5 hours for a cumflated character or companion to deflate again, this can be done pretty easily while still retaining "apple" sized or less (which is still kinda gross, but within the bounds of what Kas or a Minotaur tends to carry around). Usually what I'll do is that I'll go buy Vaush's potions and bring it up until it stops increasing volume and instead just increases testicle size (around 15 or so Virility Boosters usually), possibly also going to fuck the river Chimera a few dozen times if I don't care too much about Corruption (with Blessing those 15 boosters is only around 30 corruption, which can relatively easily be atoned if I care about it on the character, but 75 becomes problematic fast) then I complete Evergreen's quest so Meira comes home and have her max out the production via her menu, and use Incubus Cum to get Nuts Half Full and Messy Orgasm perks, at that point I feel I've done all I can legitimately do to increase things without getting into gross sizes, so at that point I export the save and edit it however much extra needed. If you don't want to cheat, then final Centaur Pack dick TF will also increase production past what the other sources will, as will Kas's Old Forest Valentine event, and finally you could get Corrupted Stud from several of Nash's super-expensive stock, which increases production and virility based on corruption, since at that point you're going to be basically DK anyway what with the other stuff.

in CoC 1, iirc, you also had ball "density" which also influenced cum production and capacity. so you could have "normal" sized balls but they'd basically be white holes (heh) of cum
