When she said that she tried to get it off by shrinking her dick why did her balls shrink? Isn't the cage just on her dick or not?

Alchemy isn't quite so precise on people not the Champ so whatever concoction Ryn took could have targeted her whole package and not just her penis whether she wanted it to or not. See Arona for the reverse example where she got balls to go with her penis in one go. Alternatively, 'cursed cage magic' made it work that way even though it was only supposed to shrink her penis. We really don't know, it's just how it worked out.
