Every so often I get emails and it says things like, "a little attachment on the mail shows" npc in a sexual pose. Is there suppose to be an actual image or is this just additional fluff content?

Those emails were implemented long before the image packs had been released. For now they are just fluff content; perhaps if suitable artwork were presented, they could be in future image packs.
