You have to introduce her to the wonder of blowjobs, then you can coach Doireann through it. I'm not sure if there's a time/number of repetitions of sex with both of them factor involved before the threesome option can proc.

Yep, get down on your knees and start sucking. I actually just tested it out with a character who hadn't done the dryad's content before and it unlocked immediately in Doireann's sex menu after seeing Orlaith's intro blowjob option. I recall it taking longer to show up with a previous Champ, but that was many patches ago so I dunno if the trigger changed or if there's a random factor involved. But I can confirm that you don't even have to have sex with Doireann on her own for the threesome to unlock.

I think you have to talk to her and select the "Orlaith" option again for the threesome to become available. I tried it last night and the threesome option was greyed out until I did that.
