Is there a way to have a abnormally large dick and a smaller one at the same time? I want to be able to access all scenes that are dependent on penis size (ie Queen Irelia: Titfuck, and Syrian: Buttfuck, the former relying on a large dick and the latter on a normal sized one). Or do I just have to choose which size I'm going to go with?

Having two penises--one large, one small--should enable these options for most scenes that search if there is a penis that fits a certain size (unless the scene is verily particular about it). As for the particulars of attaining two penises of vastly different sizes, that may vary depending on what items your character uses, if possible.
If your character is limited to one penis, then using Condensol might do the trick (temporarily shrinking a large penis by 50% or 75% it's normal length--though be careful of overdosing or there will be a permanent shrinking). Not sure if and when a growth variant of that drug will be implemented if a larger penis is desired, but that's what is possible so far.
