I get that it multiplies lust gain, but does it only affect lust gain outside of battle, only affect lust gain in battle, or both?

it does not do that. it was changed in 2023. the tooltip during character creation did not change however
Yes, it does. It still doubles passive Lust gain. If it applies to combat, I couldn't find it.
