How do you get the musk perk from lion-os?

If it's similar to getting the same perk from Laquine Ears, then you'll need a hyper penis and sufficient lion-morph score (Laquine Ears requires a penis at least 20" long and a laquine score of at least 4). I can't provide any more detail until the public GitHub is updated to the latest public build.
\*Sigh\* Of course it's male only. Or at least need a dick to get it. Is it too much to ask for a couple of musk perks for females too?
Oh well. Do you know if it stacks with the laquine one?

If it's got the same name - Musky Pheromones - then no, it won't. I haven't actually tested the TF myself, so I can't guarantee that it doesn't, but a recently fixed bug report explicitly stated Musky Pheromones, so I assume it's the same perk.
You're forgetting the Jungle Queen Scent perk from Amazona overdosing. According to the wiki it works like Pheromone Cloud, and it should stack with Musky Pheromones (though don't quote me on that).

There's also Alpha Scent, from Lupinol, which can be obtained regardless of Steele's sex. All four of these perks stack with each other for a grand total of 17 pheromone score (Musky Pheromones gives 5, the rest give 4 each), and none of them are removed if Steele changes their genital configuration, so you could just grow the necessary genitals to get the perk(s) you want, then get rid of those genitals. Or save edit them in.
I was going to mention that one, but I didn't want to bring up a male-centric version. But you're right about growing a dick then getting rid of it or save editing in order to get the wanted perk.

It's neither male-centric nor female-centric; the only requirements are having no other available TFs (which itself requires not being sexless; male, female or herm will do) or having a lupine score of at least 5 (lupine score isn't affected by genitals, so any sex can get Alpha Scent this way). Furthermore, the TF scene uses target.mf("masculine musk", "feminine fragrance") to describe part of the scent itself.

I would get the pheromone cloud too, but unfortunately it is not only male centric, it also forces brute speech which is a hard no from me
I really wish there was a way to cure all the bimbo shit because it is such a hard turnoff for me, but I also want the perks

Still though, Lion-os should give it to females too, since the TF text explicitly mentions marking partners with their scent...
There's also pheromone sweat that only requires you to have the sweaty status, Allure accessory and rutting perfume also give pheromone escore, Minbranes with enough trust will also give you pheromones if you turn those on. While not exactly counting towards pheromones score there is also flower power(which requires you to be a plant).
built a new character today and Musky Pheromones from Lion-Os is exactly the same perk from Laquine Ears but with waaaaaaaaay less RNG. Something to consider.
I am going to keep that in mind if I ever do another re-run because holy shit getting Musky Pheromones off Laquine Ears drove me insane, lol!
In response to OP, all Pheromone perks do stack provided they have different names. I have legit ever pheromone perk except Alpha Scent and Pheromone Cloud and they all tally up the pheromone level.
Speaking of, I’m a little bummed that Treated Amazons don’t have a pheromone perk like the Bulls, but Amazona overdosing provides it. I always thought it would be nice if Jungle Queen Scent could be a random perk for the Amazon Treatment path.

My "standard loadout" is jungle queen, alpha scent, pheromone sweat, and if male, laquine musky pheromones
I guess it's nice the lion-os are easier to get, and since I usually go for feline morphs anyway, it'll be way easier to get early than the laquine stuff

Except that Jade sells Laquine Ears too, and at much more manageable prices than the Lion-O's
