Am I missing something or can't healing skills be used outside of combat? In games with similar combat mechanics I can usually tell the healer to heal other characters or use my character to heal the other party members

Speaking to Cait in the Frost Hound or a camp will give you the option to have her heal your whole active party to full. Elthara offers a similar service in the Palace of Ice after completing the Winter City main quest.
Brother Sanders, Hretha can offer healing services that will heal the Champion and their party
Healing and sleep is for the weak. Real champions live off of second winds. And save scumming. and easy mode. It may not be practical, but it's honest fighting.
Do you know what else really helps being "Well Feed". Be it bacon or stew or cheese or trail rations, and if you have a Floof wife she'll make you a bento box, Oh! Joy.*

That sounds like the opposite of a problem. But seriously those bento boxes are great. If possible, get some.
Once you get to a certain point, healing becomes a non-issue. The only exception is The Glacial Rift. Jotuns hurt...a lot. Though, you can just set up a permanent camp in that temple at the northwest corner. But, yeah, camps are a thing. Alternatively, you can heal up near the end of the fight. Works wonders when you keep Cait around and use the level six healing skill.
Random chance to happen after you sleep over at her house. Maybe you need to marry her, maybe it only works if she is/isn't in the party.
Or if that doesn't work you can get a Willpower boost from Sanders
