This is mainly a pointless thing to point out, really. Not only is your last name always Steele, hence why everyone refers to you as Captain Steele, or just Steele, but Kara the NPC's full name is Kara Volke, so you cannot have the same name as another NPC by design. This isn't even a bug.

Getting pregnant despite a lack of penetration, however, is.
