Got intercepted by a void grunt out in the open of Kara's mission. Next thing I know, I get this.

I experienced this as well. Did you send Kara up he tower to cover you as I did? I'm wondering if there is some issue with her taking one of the enemies out because the similar fights inside away from her cover fire seemed to work fine.
