I've been so confused about this description for the longest time- so look, as a penis descriptor for thickness it says, 0.5/1/2 inches, and it can vary. Does this mean like going across the penis? or circumference

When refering to the thickness of a tube-like object, you usually measure from wall to wall - in other words, the diameter.
