Codex Story Drop Locations?

Hi,

Can I get a list of the stories available for the codex and their respective drop locations?

I've been grinding for 'Captured By The Demons' on Myrellion for ages, and no story code ever seem to drop on that planet.

Word of warning, though: do NOT try to edit "Becoming One of the Girls" into your inventory; it'll make the save file unloadable.
