Your appearance, inventory, key items, perks, levels and XP and your skills, status effects, and raw flags can be changed, as well as EXTREMELY small edits to certain NPCs.

The flags would be what you want to look for if you wanna change story progression. It's kinda hard and limited, but easily doable.
