Don't know if this is an known issue but some time after I gained the Ultra-Exhibitionist perk, for some reason all my clothes were unequipped and the text of the tile appeard. After this my body exposure became always at nude not matter what I equipped.

It isn't restricted to the ultra-exhibitionist perk. I'm not sure what went wrong with my savegame, but it happened to me too... and my char doesn't have that perk. However, it seems to have no effect on your ability to go places like Tavros station, or your armor bonuses (e.g. bonuses to evasion and/or resistances). It only seems to affect your Appearance description.

There are only a handful of places where this effect is created, and it's used to save us having to bother with really moving items around. The Gym on NT does this for swimwear for example.

The only other examples are with Kelly and Sera, so if you had an event with either of those there must be some way to squeak out of them without hitting the line that removes the effect. The code for gaining the Ultra-Exhib perk itself does not cause the nudity effect.

I THINK Sera might be the main cause of this now, so if either of you can confirm that you had been doing Sera content that'd go a long way to making me feel sure I might have just fixed the gap that existed.

Not unless Sera's been moved to New Texas. But since you mentioned it, I think it might've been the pool at Ten Ton Gym.
EDIT: Confirmed. I went back to New Texas, went to the pool and clicked "Rinse", and I guess it cleared the flag.

Basically I think Sera had a few exits from the scene that adds the nudity effect that missed removing it, but I fixed some other stuff involving the Gym where you could leave a room without taking an action that would clear the effect too. I think that may be the other major source of this, so with no ability to leave that room without using the Rinse option it should be handled, and you can use the pool in the gym to remove the effect if it's still stuck on you.
