At what point are the effects of the treatment determined?

Is it when you first inject yourself or is there a metaphorical die rolled at each checkpoint? I am aware that you can alter the ending based on physicality and mentality at the start, but if i were to save-scum right before injecting myself, would i get different attributes?

Both, kind of. Type (Bull, Cow, Amazon, Faux-Cow) and size targets (Horn, Breast, Penis, Balls) are set on use. At certain points, it is randomly determined whether you progress toward the size target.

what about perks? since i am going for amazon, will i get every perk for amazon or is it random? do i need to be lactating to get treated milk or will i start lactating when i take the treatment with the amazon bodytype flagged?

Most of the perks are guaranteed. You have a 50% chance of getting the 'Milkazon' status effect at 48 hours in. I've found that Cow doesn't give enough increases to Milk Multiplier to get Treated Milk from just the Treatment, as it's a one-off at a fixed time. For Amazons it's a repeated effect every hour. If I'm reading this right, the Milkazon variant is guaranteed to get it, assuming no production loss from staying too full. To be sure, just Hand Milk a few times. Or visit the Milking Barn.
