I remember Kohaku gave the player an item for 750 ec. It wasn't something from the shop, it was a yes or no question, after I bought it, I realized I didn't have enough space, so I went back to my last save (Right before buying it), but she didn't ask again. Is it a random chance or did I mess something up? (I've tried to replicate the situation 10 times).

First off, questions like this should go in the CoC2 Q&A subforum. Second: what you experienced is normal. The first time each day you enter her shop, Kohaku has a chance to offer you an item; either one you can't get anywhere else (generally not-Japanese in nature), or a powerful consumable normally exclusive to quest rewards. The item and cost are random, so you can save-scum to get a desired result. IIRC, the offer also has a one-week cooldown.
