Start to noticed npc calling my character with female pronounce instead of male so I went back to find what's going on. Found out when I do oral service for nuka nun, my gender alignment went up from 47 to 53.
Is it a bug or completely normal?

scene text states this. additionally, the scene increased your lip size by 2, which altered the appearance rating. considering the focus on using steele's mouth and the fact that she also states "The Evermind's blessing suits you well,” possibly referring to augmented lips, I am inclined to think that this was intentional.

This seems intentional--the scene adds to the character's lip size. This contributes to the calculation of the "gender weight" score (Gender Appearance Rating), so it leaves the "femininity" score (Masculinity) untouched. Hope this helps clarify.
