Here are a few minor issues I encountered when playing:
1) Azra's Tarkus Expedition - when negotiating with the Sydian Queen, the player can give up a weapon for peaceful solution. Choosing equipped weapon (ranged or melee) allows the quest to proceed, but doesn't actually remove the weapon from the player's inventory (choosing to give up any unequipped weapon from inventory works as intended).
2) Tarkus Bomb Disarming minigame - setting all lights to 'ON' doesn't register as failure (doesn't cause detonation). Similar thing on First-14 with Gravity Control and with Azra's Tarkus Expedition turret minigame - failure is impossible.
For Tarkus Expedition turret minigame, there is also a minor UI issue - setting all lights to 'ON', then clicking 'Next' and 'Reset' causes the button 'Use Gremlin' to light up - but clicking on it doesn't actually do anything.
3) Naleen Mating Ball - a trap that can be encountered during the Plantation Quest on Mhen'ga - after successful escape attempt the player is still standing at the top of the waterfall (while after victory or defeat the player is moved to the bottom and has to climb again). Also, the gangbang scene doesn't remove vaginal virginity (and is very likely to end with virgin pregnancy, and then virgin birth...).
4) Gray Goo - Quickie! (in combat sex scene) doesn't remove virginity.
5) Inverted Nipples - after using Kerokoras Venom and getting inverted nipples, 'Nipple Length' entry disappears from the Codex 'Stats' page (but nipple length is still mentioned in the 'Appearance' page, and can be modified with TFs).
6) For characters with A-cup breasts: descriptions of breasts in sex scenes are always preceded by ["tiny","girly","waifish"] - all three together, not just one adjective at random (what I'm guessing should be the intended behaviour). Descriptions of bigger breast sizes aren't as verbose. [Tested this on Masturbation-Vaginal; Celise-Female Feed; Celise-Use Her; Flahne-Get Eaten Out]
