It depends on where you sleep. If you are only sleeping at the Wayfort, then nothing happens, but if you go to Hawkthorne, either the message will pop up the moment you enter the village or you may have to sleep there one or more times. At least this is how I found out.

Seeing how PC's with a dick have scenes with Karithea and Elthara, why not have scenes for female PC's with Elthara and a bedicked Karithea (this could also lead to Elthara carrying Kari's pups).
